What Is Rehab To Home?
Rehab to Home care is generally defined as skilled nursing care that is required for a couple of days to a few months. This includes rehabilitation care following recovery from hospital surgery, from an illness or from an injury or accident. Like long term care, rehab care can be provided in a variety of settings, including a nursing home residence. What sets rehabilitation care apart from long term care is that the condition is not chronic, i.e., the individual will get better and be able to function independently again, while persons who require long term care will likely require care for the rest of their lives.
All of the services, commonly called “activities of daily living”, that are provided in long-term care are also available to the rehabilitation resident, e.g., skilled nursing care, bathing, dressing, grooming, meals, medication management, social and recreational activities and ambulatory assistance.
