Hubble Creek CCRC, a Continuing Care Retirement Community, offers both Skilled Nursing and Assisted Living in Jackson, Missouri. We are locally owned, operated and managed out of Dexter, Missouri. Our community has been newly renovated and thoughtfully modernized, blending updated design with a warm, home-like atmosphere. Every space has been created with both comfort and functionality in mind—ensuring residents who need additional support can feel safe, secure, and truly at home. At Hubble Creek, we are dedicated to delivering exceptional long-term care, assisted living, and post-acute rehabilitation services, all within one convenient campus.
Our long-term care and assisted living services are designed to provide the right level of support while promoting independence and dignity. Residents receive assistance with daily activities such as bathing, dressing, dining, medication management, and more—along with access to 24/7 nursing care and personalized attention from our compassionate team. For those recovering from illness, injury, or surgery, our short-term rehabilitation (rehab-to-home) program offers comprehensive post-acute care focused on helping individuals regain strength and safely return home. We also support residents managing chronic conditions with customized care plans tailored to their unique needs. Our in-house therapy team sets Hubble Creek apart.
